feat: default upstream balance config for new upstream form

defaultForm() now uses sub2api presets for balance fields:
- balance_endpoint: /auth/me
- balance_response_path: data.balance
- balance_divisor: 1.0

Platform presets in handlePlatformChange() remain unchanged.
